Aneurysms of the ascending thoracic aorta are a leading cause of death in Western countries and, currently in developing countries, are often underdiagnosed; the prognosis in cases of acute aortic dissection is very poor. Early diagnosis, expert medical management and preventive aortic replacement or repair surgery are key to improving long-term survival in patients with this condition. This condition often requires prophylactic surgery to resect and replace the aortic wall with a synthetic graft in order to avoid the risk of dissection or rupture. The recommendation for non-urgent surgery is based primarily on the size of the aneurysm, with a maximum aortic diameter of 4.2-5.5 cm depending on the type of valve and the patient's other risk factors. Aneurysms of the ascending aorta are predominantly of dystrophic aetiology (Marfan syndrome, aortic bicuspid valve), degenerative, inflammatory and infectious causes. High blood pressure is the main risk factor for complications.
